PLEDGE: I pledge to sponsor__________student(s) for three years of junior high school education with an annual contribution of $125/per student; _________student(s) for three years of high school education with an annual contribution of $250/per student. Students receive 100% of your contributions. We prefer your contribution paid once a year, instead of all three years in the lump sum. No parts of your contribution go to operating expenses unless you specify otherwise.

Mentor Agreement
Serve as a mentor for PEACH Foundation Financial aid Program, I agree to the following:
1. There shall not be any personal financial transactions or agreements or sponsorships between the student and the mentor unless it is recommended by PEACH Foundation.
2. The mentor shall correspond with the assigned student(s) no less than two times per year.
3. The mentor shall provide a report of the student progress to PEACH at the end of the school year, and inform the program director at PEACH of any development concerning the student that the mentor deems significant.
4. The purpose of mentor is to provide guidance appropriate for a developing student interested in academic achievement. Any discussion of a political or religious nature should be avoided.
